require long term care and support. Presently our home is centrally located 3-5 minutes drive from the Half Way Tree square. Our address at 35 B Waterloo Avenue is easily accessible from Half Way tree via Waterloo Road as the first left turn after Devon House entrance. 35 B Waterloo is at the very end of the avenue. The building is a modern duplex type setting which provides a spacious, refreshing and comfortable environ.
STAFF Our staff consists of a doctor, a registered nurse, seven practical nurses and two general workers. Dr. Andrew Greene brings to MonaCare more than 15 years extensive experience as a family physician. His knowledge and experience in caring for the elderly has made him ideally suited as the in house physician ultimately responsible for the total care and support for our family of residents. Nurse Nadene Richards is our registered nurse assigned to supervise the entire nursing staff. Nurse Richards visits the home at scheduled days and times, during which all aspect of patient-care plan, medical supervision and management of the nursing home is reviewed and implemented. The day to day operation and management of the nursing home is entrusted to the most capable nurse Beverly Nicholas. Nurse Nicholas brings to MonaCare years of experience as a practical nurse. She is not only involved in the direct day to day management of the nursing home but she also assist with the training of new student nurses who occasionally rotates through our home for that ever so valuable experience. Nurse Nicholas is pleasant, friendly and well known by all. Relatives and visitors to the home will undoubtedly meet and interact with Nurse Nicholas as she is always available to provide the latest update on all the residents state of health or well being.
SERVICES MonaCare Nursing Home is a facility offering care for the elderly convalescent patient or elderly person seeking excellent long term accommodation and medical care. Provided the space is available, and following an assessment by the in house doctor, we are able to care for critically ill patients. To a limited extent we also accept day visits for those elderly persons who live at home but require day care services from time to time. The following services are also offered or pending:- • Placement of experienced practical nurses • Training and certification of Geriatric practical nurses • Home care services by medical doctor and or practical nurses VISITING HOURS Immediate relatives of residents may visit at any time. However to facilitate the nursing staff with their duties of baths, feeding and personal care of residents we ask that extended visits are made at the most convenient hours.
10am to 12noon
6pm to 8pm
We also ask that unavoidable visits outside of these times should follow a phone call to the home so as to alert the nursing staff. ADMISSIONS & FEES All residents will be provided with three full meals, plus a minimum of one snack per day. Other snacks may be provided by relatives. Particular emphasis is placed on the patients nutritional needs and residents / relatives are required to complete a nutritional questionnaire before admission.
Before patients are admitted to the home relatives are given a personal items check list outlining the minimum personal items required at admission. General fees Our fees are very reasonable. Long term stay, short term stay or for special care } please contact home for current details of fees. An Additional fee is charged for “Special care”. This applies when patients require any of the following medical care - The Comatose patient - Daily dressings - Naso-gastric tube feeding - Physiotherapy. - Other cases may apply Admission check list - Admission Agreement Contract. - Medical Evaluation & Referral report - Nutritional Profile - Personal Items List We maintain a medical file on all residents. In order to create an up-to-date file, relatives are required to obtain a detail medical report summary or referral letter from their family doctor. Alternatively a medical evaluation can be done by our in-house doctor (a fee applies) at MonaCare Medical services. TERMS AND CONDITIONS The home has a standard terms and condition document that all new residents / sponsoring relatives are required to read and agree to comply with. This document outlines the services provided, fees charged and the requirements of the resident. It addresses the issue of late payment of fees, discounts and refunds. This document is only available at the nursing home. Upon reading this document relatives / sponsoring relative will then be required to sign an "admission agreement contract" which is also only available from the nursing home. MEDICAL CARE As a special feature to this nursing home there are no additional charges for consultation and care by our in-house medical doctor. All residents will receive regular checks. Naturally relatives will be responsible for all medications prescribed. In the event of a medical illness, acute or on-going, this will be managed by our in-house doctor at no additional charge. CASES REQUIRING SECONDARY OR SPECIALIST CARE will be referred to the necessary institutional / doctors, the cost of which will be the responsibility of the resident / sponsor. Residents may continue under the care of their family physician or clinic as arrangements can easily be made to accommodate this. The in-house doctor will coordinate any interface with outside doctors and institutions. Residents are responsible for all personal and medical disposables. OCCUPATIONAL ACTIVITIES AND SUPPORT The home is committed to the concept of continued mental stimulation and offer occupational / recreational activities for our elderly residents. Our activities include morning devotions, bible readings with nurses, television and radio programs, writings and art work sessions, board and card games activities and "mass" on Sundays. Support groups:- All residents have the option to become a part of the government’s drug for the elderly program. Church groups and other senior citizens organizations are an essential part of social interaction in this nursing home and we receive regular visits from these groups.
